Dannie Davis IV Killed in Two-Vehicle Crash on State Route 15 in Putnam County
PUTNAM COUNTY, Ohio — A Wednesday morning crash in Putnam County claimed the life of a Lima man and left another driver seriously injured, according to the Ohio State Highway Patrol.
Troopers said the fatal two-vehicle collision occurred at approximately 6:38 a.m. on May 13, 2026, along State Route 15 near milepost 4 in Monroe Township.
Preliminary findings from the Ohio State Highway Patrol indicate that a 33-year-old man from Lima, later identified as Dannie Davis IV, was driving a Honda Accord westbound on State Route 15 when the vehicle traveled left of center and collided head-on with an eastbound Infiniti G37.
Authorities said the Infiniti was being driven by a 25-year-old man from Bryan, Ohio.
Emergency crews responded quickly to the scene, but Davis suffered fatal injuries in the violent collision and was pronounced dead at the scene.
The driver of the Infiniti sustained serious injuries and was transported to a nearby hospital for emergency medical treatment. Officials have not released an update on his condition.
Investigators confirmed that both drivers were wearing seatbelts at the time of the crash.
According to the preliminary crash investigation, drug impairment is suspected to have been a contributing factor in the collision, although authorities have not yet released additional details pending further investigation and toxicology results.
The Ohio State Highway Patrol continues to investigate the deadly crash.
Troopers are also reminding motorists about the dangers of impaired driving and urging drivers to never operate a vehicle while under the influence of drugs or alcohol.

Train Derails in Ferguson Involving Vehicles, Hazmat Team Called Over Ethyl Alcohol Rail Car
FERGUSON, Mo. — A train derailment involving vehicles prompted an emergency response in Ferguson on Ferguson Avenue, where authorities requested a hazardous materials team after determining that one of the train cars contained ethyl alcohol.
Emergency units were dispatched to 617 Ferguson Avenue after reports of the rail incident. Responders found a train that had derailed with vehicles involved.
Officials confirmed there was no reported entrapment. Crews were checking the scene for injuries and evaluating those involved in the incident.
A Hazmat response was requested because one rail car involved in the derailment was carrying ethyl alcohol. Emergency crews were assessing the rail car for any potential leak or hazardous condition and working to secure the area.
The presence of ethyl alcohol prompted additional safety precautions while responders examined the derailed train and surrounding vehicles.
At this time, authorities have not released details about possible injuries, the cause of the derailment or the extent of damage.
The investigation and emergency response remain ongoing in Ferguson, and officials are expected to provide additional information once the scene has been fully assessed.
